Dear Parent/Guardian,
We are thrilled to invite your child to participate in the 2025-2026 L.E.A.D. after school program. This program showcases the creativity and talent of young artists in our community, providing them with a platform to share their work with a wider audience while enhancing leadership skills.
We kindly request your permission to photograph and print your child's artworks for display.
PERMISSIONS:
1. Display Permission:
I hereby grant permission for my child & artwork to be photographed. I understand that these images may be used in promotional materials related to the program & exhibit but not limited to brochures, posters, and social media posts.
AGREEMENT:
By checking this box, I affirm that I am the parent/legal guardian of the minor named above and have the legal right to consent to and, by signing, I do consent to the terms and conditions described. I release Gaylord Area Council for the Arts and its agents of the Leadership Empowerment through Artistic Development Program from any claims, demands, and causes of action in connection with this program and Art works described in this permission form.
